Mediation: How to Get Started

Please complete the Mediation Intake Form and have your partner do the same. The intake
form contents are confidential and will NOT be shared with the other party. The intake form
can be obtained by completing the Consultation Request Form on the Heft Law website. An
intake form will be forwarded to you promptly.

We do not ask the other party for their form, we let each party submit the form on their own,
when they are ready. We will not open a file until both forms are received.

All forms are to be submitted to info@heftlaw.ca.

The intake form helps us prepare for your individual meeting. At the initial individual meeting,
we will ask you questions and you can ask us questions. This helps us decide if mediation is right
for your situation.

After the individual meetings, you and your partner will need to sign our Agreement to
Mediate. The Agreement to Mediate should be reviewed carefully and before your individual
meeting in case you have questions.

We will ask both parties to make an appropriate deposit prior to the individual meetings.
